AFC Care Manager

Immediate opening for a full-time (37.5 hours), benefitted position. This care manager is a vital part of our AFC "Adult Foster Care" position that allows individuals who need assistance with daily living activities to live with qualified caregivers. The care manager provides training and support services and oversight of those caregivers and does require a bachelor's degree. As the Care Manager is required to travel to the homes of the caregivers, they must have access to a vehicle and a valid driver's license. Role includes mileage reimbursement, tuition remission program, health insurance, paid leave and a retirement program with agency match after one year/1000 hours.

Family Support Advocate

Long-term recruiting effort for both full-time (37.5 hours) and part-time roles. Rates starting at $16.00 per hour and increases with education, experience and/or contract requirements. UCP advocates are dedicated and passionate about working with individuals with disabilities in the local community. Our focus is on community inclusion and socialization through a variety of activities that take into account the interest and life skill needs of each member, such as shopping, cooking and supporting members in their hobbies. As the Advocate is required to travel to the homes of the members, they must have access to a vehicle and a valid driver's license. Role includes mileage reimbursement, a stipend program, tuition remission program, health insurance (30 hours per week), paid leave and a retirement program with agency match after one year/1000 hours. You must be at least 18 years old and have some familiarity with the needs of individuals with disabilities, gained through either or both life, work experience.

In Home Support

Long-term recruiting effort for both full-time (37.5 hours), part-time and per diem opportunities. In Home Support provides customized support to individuals with disabilities to live a more meaningful home and community life. This role pays $15.00 per hour. Role includes mileage reimbursement, tuition remission program, sick leave and a retirement program with agency match after one year/1000 hours. Staff working 30+ hours per week also have access to a full complement of agency benefits including medical insurance.
